I believe there should be a steady balance of both, as those cannot afford healthcare should get a free pass by the government, for example, unemployment, disability patients. As for those who can afford it, should pay where they can as this helps perpetuate economy as well as addresses specific needs of the patients. Some medical issues require specialist facilities to further medical research in situations where governments are not allowed to act on, due to regulations
