In a single-payer system all residents receive healthcare regardless of age, income or health status. I think that while it is a good idea, people who don't often go to the hospital aren't paying for their own hospital stays but for others and while that is ok, I think that people should be able to opt out and if they do then they pay for their own hospital fees.
